Place your order by faxing the order form below to (800) 323-2225. Please ensure that you have printed your order information so that it is legible. You will receive an order confirmation by email once your order is entered.

We ship all orders from our Jacksonville, Fla. facility, usually within 24 hours. We must charge exact shipping charges for orders to be sent outside of the continental United States. Please call us with your order to be delivered to Alaska, Hawaii or other countries. For orders shipping to the continental U.S., shipping charges are $10.00 per order, plus $2.00 per product ordered and shipped to states in the Eastern and Central time zones OR $3.00 per product ordered and shipped to the Mountain and Pacific time zones.

Customer satisfaction is always our No. 1 goal. If you are dissatisfied with your purchase, we will happily issue a replacement or a full refund if you notify us within 30 days of purchase. Please contact our Customer Service team at 800-874-7715 for instructions to ensure proper credit. We ask that you return any merchandise in its original packaging along with the packing slip.

We accept Mastercard, VISA, Discover, Diners Club and American Express credit cards, debit cards, and personal checks. There is no surcharge for the use of any credit card.

Cash on Delivery is available. There is no COD service charge for orders over $150. Call Bodyline at 800-874-7715 if you wish to pay for your order with a COD payment.
